Some pickups have a 5% chance of being rerolled into rarer pickups:
• Any heart other than Eternal Hearts/ Black Hearts/ Bone Hearts/ Gold Hearts that spawns in the future will be rerolled into one of them when spawned.
• Any coin other than Lucky Pennies/ Golden Pennies that spawns in the future will be rerolled into one of them when spawned.
• Any bomb other than Giga Bombs/ Golden Bombs that spawns in the future will be rerolled into one of them when spawned.
• Any key other than Charged Keys/ Golden Keys that spawns in the future will be rerolled into one of them when spawned.
• Any battery other than Mega Batteries/ Golden Batteries that spawns in the future will be rerolled into one of them when spawned.
• Any pill that spawns in the future will be their golden variant when spawned.
